To qualify for the CDHCI program, you must first receive a home care assessment through Alberta Health Services (AHS). If you live in Medicine Hat, simply contact Medicine Hat Home Care and request an assessment. During the assessment, ask whether you may qualify for the CDHCI program under Alberta Blue Cross.

AHS will review your care needs, confirm eligibility, and determine the number of hours you can receive through the program.
